彩超指导脐带绕颈分娩前干预策略及效果观察

目的：探讨分析彩超指导脐带绕颈分娩前干预策略及效果。方法：选取2012年7月-2013年6月在本院接受治疗的200例脐带绕颈的产妇，按照随机数字表法将其分为对照组和观察组各100例。对照组不采用分娩前干预措施，观察组采用超声检查对产妇脐带绕颈类型与方向进行确定，针对绕颈类型与方向，通过产妇卧姿，对其胎心与产程变化进行观察，以此选择合适的分娩方式。观察比较两组产妇分娩方式和胎儿宫内窘迫发生率及新生儿窒息率。结果：观察组的自然分娩率明显高于对照组，剖宫产率和胎儿宫内窘迫发生率均明显低于对照组，且新生儿窒息率明显低于对照组，差异均有统计学意义（P〈0.05）。结论：彩超指导脐带绕颈分娩前干预措施能够有效降低胎儿宫内窘迫发生率、剖宫产率以及新生儿窒息率，值得临床应用与推广。

Intervention Strategies and Effect Observation of Color Doppler Ultrasound Guiding Umbilical Cord Around Neck before Delivery

Objective：To investigate and analyze the intervention strategies and effect of color doppler ultrasound guiding umbilical cord around neck before delivery.Method：200 maternal with umbilical cord around the neck in our hospital for treatment from July 2012 to June 2013 were selected,they were randomly divided into the observation group and the control group,100 cases in each group.The control group was not given intervention measures before delivery, the observation group was determined the maternal umbilical cord around neck type and direction,according to the type and direction around the neck,through maternal prone position,the fetal heart and the production process changes were observed,and selected the appropriate mode of delivery.The maternal mode of delivery,the incidence of fetal distress and neonatal asphyxia between the two groups were observed and compared.Result：The natural birth rate of the observation group was significantly higher than the control group,the incidence of cesarean section and fetal distress were significantly lower than those of the control group,and the rate of neonatal asphyxia was significantly lower than the control group,the differences were statistically significant（P〈0.05）.Conclusion：Intervention measures of color doppler ultrasound guiding umbilical cord around neck before delivery can effectively reduce the incidence of fetal distress,cesarean section rate and neonatal asphyxia rate,it is worthy of clinical application and promotion.
