地诺前列酮对足月胎膜早破宫颈成熟及母婴结局的影响

目的：观察地诺前列酮对足月胎膜早破宫颈成熟及母婴结局的影响。方法118例足月胎膜早破孕妇按照数字表法随机分为两组,两组各有59例。对照组采用催产素治疗,观察组采用地诺前列酮治疗。观察两组12 h促宫颈成熟程度,剖宫产、产褥感染、产后出血、新生儿肺炎及胎儿窘迫发生情况。结果对照组显效27例,有效23例,无效9例,总有效率为84.7%；观察组分别为32例,24例,3例,总有效率为94.9%。两组比较差异有统计学意义（χ2=4.86, P〈0.05）。对照组剖宫产、产褥感染、产后出血、新生儿肺炎及胎儿窘迫为27例、13例、（492.1±177;38.4）ml、6例、4例；观察组分别为6例、4例、（301.1±177;33.7）ml、2例、1例。观察组均明显少于对照组（χ2=6.94,χ2=5.78, t=3.0692,χ2=4.72,χ2=5.06, P〈0.05）。结论±对足月胎膜早破产妇采用地诺前列酮治疗可以明显提高宫颈成熟程度,减少产后母婴并发症。

Impact of dinoprostone on cervical ripening and maternal outcomes in patients with premature rupture of membranes

Objective To study the impact of dinoprostone on cervical ripening and maternal outcomes in patients with premature rupture of membranes. Methods 118 patients with premature rupture of membranes were randomly divided into two groups, 59 cases in each group. The control group was treated through oxytocin, while the observation group was treated through dinoprostone. Two groups were observed about 12 hours of cervical maturity, cesarean section, puerperal infection, postpartum hemorrhage, neonatal pneumonia and fetal distress situation. Results The control group had 27 cases markedly effective, 23 cases effective, 9 cases no-effective, the effective rate was 84.7%. The observation group were 32 cases, 24 cases, 3 cases, the effective rate was 94.9%. There were significant differences between the two groups （χ2=4.86, P＆lt;0.05）. Cesarean group, puerperal infection, postpartum hemorrhage, neonatal pneumonia and fetal distress were 27 cases, 13 cases, （492.1＆#177;38.4） ml, 6 cases, 4 cases in the control group. The observation group were 6 cases, 4 cases, （301.1＆#177;33.7） ml, 2 cases, 1 case. The observation group was significantly less than the control group （χ2=6.94,χ2=5.78, t=3.0692,χ2=4.72,χ2=5.06）. Conclusion Mothers of premature rupture of membranes used dinoprostone cervical therapy can significantly improve the degree of maturity, reduce postpartum maternal complications.
