经颅彩色多普勒对脑血管疾病的筛查探讨

目的：探讨TCD应用于脑血管疾病筛查的应用价值，为患者提供预防和治疗的指导。方法：应用HDI-3000型GEvivid7型彩色多普勒超声诊断仪，经枕窗检测BA、VA，经颞窗检测ACA、MCA、PCA，使用2 MHz脉冲探头，探测血管的血流方向、血流速度、频谱形态、脉动指数等。结果：TCD颅内动脉成功显示率随着年龄的增长而不断降低，50岁以下的显示率超过80%，而〉51岁者显示率约为65%以下，两者比较差异有统计学意义（P〈0.05）。240例受检者中，脑血管疾病总检出72例，占30.0%；其中动脉硬化患病筛出率14.58%；BA检出率5.42%；VA检出率4.17%；MCA和ACA检出率均为2.08%；PCA检出率最低为1.67%。结论：TCD查体简单无创、快捷方便，能够准确地筛查出脑血管疾病发生、发展情况，对脑血管疾病的早发现、早预防、早治疗具有积极的意义。

Discussion of Transcranial Color Doppler Screening for Cerebrovascular Disease

Objective：To investigate the application value of TCD used in screening of cerebral vascular diseases, and provide prevention and treatment guidelines for patients.Method：HDI-3000 type GEvivid7 color doppler ultrasonic diagnostic apparatus was applied,BA and VA were detected through pillow window,ACA,MCA and PCA were detected through temporal window,2 MHz pulse probe was used to detect vascular flow direction,flow velocity,spectral shape,pulsating index.Result：With the growth of age,the displayed success rate of TCD intracranial artery continually decreased.The displayed success rate of under the age of 50 years old was more than 80%,while the displayed success rate of more than 51 years old was about less than 65%,the difference was statistically significant（P〈0.05）.240 cases was checked,the total detection of cerebrovascular disease were 72 cases,accounting for 30.0%.prevalence of atherosclerosis which screen out rate was 14.58%;BA detection rate was 5.42%;VA detection rate was 4.17%;MCA and ACA detection rate were 2.08%;PCA lowest detection rate was 1.67%.Conclusion：TCD examination is simple non-invasive,fast and convenient,can accurately screening out cerebrovascular disease,the development of cerebrovascular disease early detection,early prevention and early treatment are positive meaning.
