老年女性慢性充血性心力衰竭血红蛋白水平变化及其对预后的影响

目的探讨老年女性慢性心力衰竭（CHF）患者血红蛋白（Hb）水平及其对预后的影响。方法入选78例60岁以上老年女性,均符合纽约心脏病学会（NYHA）分级Ⅱ～Ⅳ级的心力衰竭患者。对不同心功能级别的心力衰竭患者的贫血发生率和Hb水平及血清肌酐水平进行比较分析。随访1年,对贫血组和非贫血组随访期间的心血管事件发生情况进行比较。结果Hb水平随着NYHA分级的升高而降低,贫血发生率随着NYHA分级的升高而呈升高趋势;患者血清肌酐随着NYHA分级的升高逐渐升高。随访期间,贫血组和非贫血组心力衰竭加重的患者比例、平均住院次数≥2次的患者比例、平均住院时间≥30d的患者比例比较,差异有统计学意义。结论在老年女性CHF患者中贫血的发生与心力衰竭的严重程度相关,在治疗心力衰竭的同时,应积极纠正贫血。

Change of Hemoglobin Level in Female Elderly Patients with Chronic Congestive Heart Failure and Its Influence on Prognosis

Objective To study the change of hemoglobin(Hb) level in female elderly patients with chronic congestive heart failure (CHF) and its influence on prognosis.Methods Totally 78 female patients aged ≥ 60 years with a principal diagnosis of chronic heart failure (CHF) were included.The incidence of anemia,HP level,and serum creatinine (CR) level were compared among patients with different cardiac functional grades.and the incidences of cardiovascular events were compared between anemia group and non-anemia group during one-year follow-up.Results Along with the development of cardia insufficiency,Hb level decreased,the incidence of anemia increased,and CR level increased.Patients with anemia had significantly higher re-hospitalization rate and longer hospital stay than non-anemia patients during follow - up.Conclusion Anemia is closely correlated with the severity of CHF.Therefore,anemia should be actively treated during CHF management.
