埃索美拉唑、左氧氟沙星、阿莫西林三联法治疗幽门螺杆菌感染疗效观察

目的探讨埃索美拉唑＋左氧氟沙星＋阿莫西林-周疗法根除幽门螺杆菌（HP）的疗效。方法选择82例HP阳性消化性溃疡患者．随机分为两组。治疗组采用埃索美拉唑（20mg，每日2次）加左氧氟沙星（200mg，每日2次）加阿莫西林（1000rag，每日2次），治疗7天后继续用埃索美拉唑20mg，每日1次，3周。对照组采用埃索美拉唑（20mg，每日2次）加甲硝唑（400mg，每日2次），加阿莫西林（1000mg，每日2次），治疗7天后继续用埃索美拉唑20mg，每日1次，3周。疗程结束4周后复查胃镜和HP，观察HP根除率，症状缓解率、溃疡愈合率和不良反应。结果治疗组和对照组HP根除率分别为92．7％和75．6％，两组差异有显著性（P〈0．05），治疗组和对照组症状缓解率、溃疡愈合率和不良反应发生率分别为95．1％和90．2％，95．1％和92．7％，22％和19．5％，两组无明显差异（P〉0.05）。结论埃索美拉唑加左氧氟沙星加阿莫西林三联1周疗法是根除HP的理想方案。

Triple therapy of esomeprazole,levofloxacin and amoxicillin combined for Helicobacter Pylori eradication

Objective To study the efficacy of 1 week triple therapy of esomeprazole, levofloxacin and amoxicillin combined for Helicobacter pylori(Hp) eradication. Methods 82 patients with Hp infection Suffering from Peptic ulcer were randomized to treatment group and control group. The treatment group were administered esomeprazole (20mg,twice a day) , levofloxacin (200mg, twice a day) and amoxicillin (1 000mg, twice a day) for 1 week . The control group were administered esomeprazole (20mg,twice a day), metronidazole (400mg, twice a day) and amoxicillin (1 000mg, twice a day) for 1 week . All patients were treated with esomeprazole (20mg,every day)for another three weeks. Four weeks later, the Hp eradication, symptoms, peptic ulcer healing and side-effects were observed. Results The Hp eradication rate between the treatment group and control group were 92.7%, 75.6%, respectively. There was significant difference between two groups (P<0.05). The symptom ameliorate rate, puptic ulcer healing rate and side-effect rate between the treatment group and control group were 95.1% and 90.2% , 95.1% and 92.7% , 22% and 19.5% , respectively . The difference was not significant (P>0.05 ). Conclusion 1 week triple therapy of esomeprazole, levofloxacin and amoxicillin combined is the first choice for Hp eradication.
