胚胎培养液中氨基酸浓度对选择性单胚胎移植妊娠结局的影响

摘    要:目的探讨体外受精-胚胎移植(in vitrofertilization-embryo transfer,IVF-ET)过程中胚胎培养液中氨基酸浓度与选择性单胚胎移植妊娠结局的关系。方法 50例接受选择性单胚胎移植患者,收集胚胎移植当日的胚胎培养液50份,根据妊娠结局分为妊娠组与未妊娠组,采用高效液相色谱法检测培养液中20种氨基酸含量,分析培养液中氨基酸浓度与选择性单胚胎移植后妊娠结局的关系。结果临床妊娠者17例,妊娠成功率为34.00%;妊娠组胚胎培养液中甘氨酸和丙氨酸浓度((67.5±8.9)、(62.0(15.0,83.6)μmol/L)明显低于未妊娠组((85.7±65.0)、(93.2(21.2,124.3)μmol/L),差异均有统计学意义(P〈0.05);校正混杂因素后,胚胎培养液中甘氨酸(OR=0.980,95%CI:0.970-0.990,P=0.025)和丙氨酸(OR=0.990,95%CI:0.980-1.000,P=0.018)浓度与妊娠结局呈负相关。结论胚胎培养液中甘氨酸和丙氨酸浓度与选择性单胚胎移植是否妊娠有关;2种氨基酸浓度较低时妊娠成功率较大。

Abstract:Objective To explore the relationship between amino acid concentration in embryo culture medium and the outcome of elective single embryo transfer(eSET)in in vitro fertilization-embryo transfer(IVF-ET).Methods A total of 50 patients undergoing eSET were collected 50 copies of embryo culture medium on the day of IVF-ET and were divided into pregnancy group and non-pregnancy group according to the pregnant results. High performance liquid chromatography was used to detect the concentrations of 20 kinds of amino acid of embryo culture medium.The relationship of amino acid concentration of embryo culture medium and the outcome of eSET was analyzed.Results Seventeen patients were clinically pregnant(34.00%).The concentrations of glycine and alanine of the embryo culture medium in the pregnancy group((67.5±8.9),62.0(15.0,83.6)μmol/L)were lower than those in non-pregnancy group((85.7±65.0),93.2(21.2,124.3)μmol/L)(P〈0.05).After adjusting confounders,the concentrations of glycine and alanine were negatively correlated with pregnant outcome(OR=0.980,95%CI:0.970 to 0.990,P=0.025;OR=0.990,95%CI:0.980 to 1.000,P=0.018).Conclusion The concentrations of glycine and alanine in the embryo culture medium are correlated with pregnancy of eSET,and lower concentration of glycine and alanine in the embryo culture medium predicts a high pregnancy rate.
