自体脂肪颗粒移植术中脂肪栓塞的预防

目的探讨自体脂肪颗粒移植术治疗面部凹陷畸形的疗效及脂肪栓塞的预防方法。方法面部局部凹陷畸形患者123例,均于局部麻醉下行自体脂肪颗粒移植修复术,观察术后效果及并发症发生情况。结果 35例手术一次成功,88例经2-4次手术填充;随访3-16个月,其外形均有不同程度改善,术中及术后无脂肪栓塞发生。结论自体脂肪颗粒移植术治疗面部凹陷畸形疗效确切;规范脂肪细胞取材、富集和自体脂肪注射移植操作,是预防血管和脂肪细胞损伤、防止脂肪栓塞的关键。

Intraoperative prevention of fat embolism during autologous fat transplantation

Objective To explore the effect of autologous fat transplantation and the prevention of fat embolism.Methods Autologous fat transplantation was performed under the local tumescent anesthesia to repair the facial depression in 123 cases.The therapeutic effect and complications were observed.Results Of these 123 cases,preliminary success was achieved in 35,and the other 88 cases underwent twice to four times implantations.During the follow-up survey for 3to 16 months,all patients were satisfied with the appearance.No fat embolism was found in the whole therapeutic process.Conclusion Autologous fat transplantation is effective for facial depression.The prevention methods such as performing and normalizing the procedures of adipose tissue harvest,enrichment and autologous injection strictly and comprehensively,are crucial to the prevention of vascular trauma,adipose cell trauma and fat embolism in autologous fat transplantation.
