脂肪衰减参数在脂肪肝诊断中价值及相关因素分析

摘    要:目的探讨脂肪衰减参数对脂肪肝的诊断价值,评估脂肪肝相关因素与脂肪衰减参数的关系。方法检测脂肪肝患者128例(脂肪肝组)和非脂肪肝患者113例(对照组)脂肪衰减参数,再依据脂肪衰减参数将241例患者分为脂肪衰减参数升高组122例和脂肪衰减参数正常组119例,分析脂肪衰减参数与年龄、性别、体质量指数的关系。结果脂肪肝组与对照组、脂肪衰减参数升高组与脂肪衰减参数正常组性别比例差异均无统计学意义(P〉0.05),脂肪肝组年龄((40.67±11.83)岁)、体质量指数((26.65±3.41)kg/m^2)以及脂肪衰减参数((264.30±28.26)db/m)与对照组((36.73±12.61)岁、(22.30±3.45)kg/m^2、(217.62±15.72)db/m)比较差异有统计学意义(P〈0.05);脂肪衰减参数升高组年龄((40.02±12.26)岁)、体质量指数(26.70±3.40)kg/m^2)均大于脂肪衰减参数正常组((37.16±12.54)岁、(22.54±2.75)kg/m^2))(P〈0.05);脂肪衰减参数诊断脂肪肝的敏感度为89.06%,特异度为95.58%。结论脂肪衰减参数可准确评估肝脏脂肪含量,年龄、体质量指数与脂肪衰减参数有关。

Abstract:Objective To evaluate the value of fat attenuation parameter(FAP)to the diagnosis of fatty liver disease,and analyze its correlation with the relative factors of fatty liver disease.Methods A total of 241 patients including 128 patients with fatty liver disease(fatty liver group)and 113 patients without fatty liver disease(control group)were detected FAP,and were redivided into high FAP group(n=122)and normal FAP group(n=119).The relationships of FAP with gender,body mass index(BMI)and age were analyzed among groups.Results There were no significant differences in the gender ratio between fatty liver group and control group,and between high FAP group and normal FAP group(P〈0.05).The age,BMI and FAP were(40.67±11.83)years,(26.65±3.41)kg/m2 and(264.30±28.26)db/m in fatty liver group,significantly different from those in control group((36.73±12.61)years,(22.30±3.45)kg/m^2,(217.62±15.72)db/m)(P0.05).The age and BMI were(40.02±12.26)years and(26.70±3.40)kg/m^2 in high FAP group,significantly higher than those in normal FAP group((37.16±12.54)years,(22.54±2.75)kg/m^2))(P〈0.05).The sensitivity and specificity of FAP for fatty liver were 89.06% and 95.58%.Conclusion FAP can be used to evaluate accurately the content of fat in liver,and is correlated with the age and BMI.
