摘    要:目的:探讨初学者应用改良的拦截-劈核技术在硬核白内障超声乳化手术中的应用效果。方法:对36例(36眼)Ⅳ级以上硬核白内障采取改良的拦截-劈核技术,把晶状体硬核分成6-8块碎核,逐个乳化吸除小块碎核。观察手术时间、并发症、术后视力、术后角膜水肿发生率等指标。结果:术中平均超声乳化时间为(181.8±39.21s,有效超声乳化时间为(52.2±9.8)s。术后第1天矫正视力≥0.8者14眼,≥0.5者28眼,≥0.3者30眼。≤0.2者6跟。术后1周裸眼视力≥0.8者16眼,≥0.5者30眼,≥0.3者33眼,≤0.2者3眼;术中后囊破裂2眼,角膜切口灼伤6眼;术后角膜水肿6眼。结论:采用改良的拦截-劈核技术进行硬核白内障手术,可减少手术并发症、缩短学习曲线,值得推广应用。

Application of the modified stop and chop technique in phacoemulsification for hard nucleus cataract

Abstract:Objective: To investigate the effect of the modified stop and chop method in phacoemulsification for hard nucleus cataract. Methods: The modified stop and chop technique was performed in 36 cases (36 eyes) of cataract with hard nucleus graded Ⅳ and V. The hard nucleus was divided into 6 to 8 fragmentations, and all fragmentations wereremoved one by one using phaeoemulisification. To observe phacoemulsifieation time, operative complications, postoper-ative vision, and corneal edema. Results: The averaged time of phacoemulisifieation is(181.8±39.2) seconds, and aver-aged utility phacoemuslisification time is (52.2±9.8) seconds. At one day postoperatively, the corrected vision was 0.8 or better than it in 14 eyes ,0.5 or better than it in 28 eyes, 0.3 or better than it in 30 eyes. Less than 0.2 in 6 eyes. At 1 week postoperatively, the corrected vision was 0.8 or better than it in 16 eyes ,0.5 or better than it in 30 eyes, 0.3 or better than it in 33 eyes. Less than 0.2 in 2 eyes.The major complications were posterior capsule burst, corneal edema and corneal incision heat injury. Condusion: To a beginner, the modified stop and chop technique in phacoemulsifiea-tion for hard nucleus cataract is favored because of its effectiveness, safety, easy manipulation, and less time consuming and fewer complications.
