p53和Ki67在胃癌中的表达及其临床意义

目的探讨p53蛋白、Ki67在慢性胃炎和胃癌中的表达及意义。方法采用免疫组化法分别对58例胃癌,20例胃炎,进行了p53蛋白和Ki67的检测。结果①胃癌组织中p53蛋白、Ki67的阳性表达率分别为31.03%(18/58)、70.69%(41/58),均显著高于胃炎组织,差异均有统计学意义(P=0.042,P=0.005)。②在胃癌组织中p53蛋白的阳性表达与胃癌的临床分期、浸润深度、和淋巴结转移有关(P<0.05),与组织学分化程度无关(P>0.05);胃癌中Ki67的表达阳性率与胃癌的临床分期和浸润深度有关(P<0.05),与组织学分化程度和淋巴结转移无关(P>0.05)。③p53蛋白和Ki67共同阳性者14例,共同阴性者14例,相关性分析显示,二者之间呈正相关(r=0.372,P=0.004)。结论 p53蛋白和Ki67在胃癌中的表达明显升高,可作为判断胃癌恶性程度及预后的重要参考指标。

Expression of p53, Ki67 in Gastric Carcinoma and Clinical Significances

Objective To investigate the expression of protein p53 and Ki67 in gastric carcinoma and its prognostic significance. Methods The expression of p53 and Ki67 in gastric carcinoma of 58 cases and chronic gastritis of 20 cases were detected through immunohistochemistry, respectively. Results （1） The expression rate of protein p53 and Ki67 in 58 cases of gastric carcinoma were 31.03%（18/58）and 70.69%（41/58）, respectively, which were significantly higher than that in chronic gastritis.（P=0.042, P=0.005）. （2）Ingastric carcinoma, the expression of protein p53 was significantly associated with the.depth of invasion（P=0.000）, lymph node metastasis（P=0.018） and clinical staging（P=0.001）. It was not associated with the degree of differentiation（P〉0.05）. The expression of Ki67 was significantly associated with the depth of invasion（P=0.015） and clinical staging（P=-0.043）. But it was not associated with the degree of differentiation and lymph node metastasis（P〉0.05）. （3）The positive expression of protein p53 and Ki67 were 31.03% and 70.69%, respectively, in which co-positive in 14 cases and the co-negative in 14 cases. The protein p53 was correlated with the Ki67 expression（r=0.372, P=0.004）. Conelution The expression of p53 and Ki67 were higher in the gastric carcinoma than that in chronic gastritis. Therefore, the detection of protein p53 and Ki67 expression could be used as biomarkers to predict the degree of malignancy and prognosis of gastric carcinoma.
