丙泊他莫用于剖宫产术后镇痛对泌乳素的影响

目的观察剖宫产术后应用丙泊他莫持续镇痛对产妇泌乳素的影响。方法选择剖宫产孕妇120例，随机分为三组，每组各40例。1组采用丙泊他莫2g＋1．6％利多卡因12mL＋生理盐水至100mL入泵，2组采用吗啡6mg＋盐酸异丙嗪12．5mg＋1．6％利多卡因12mL＋生理盐水至100mL入泵。此两组术后均给予电子泵持续硬膜外镇痛。3组不给予硬膜外镇痛，分别于麻醉前、术后24h、48h监测泌乳素的含量，观察术后不良反应。结果术后24h、48h，1、2组血清中PRL明显高于3组（P〈0．01），但1、2组对比无明显差异（P〉0．05）。不良反应1组明显低于2、3组（P〈0．05）。结论丙泊他莫术后持续镇痛对泌乳素影响小，有利于乳汁的分泌。

Effect of PRL Analgesia after Cesarean Section on Prolactin

Objective To observe the effect of Propacetamol Hydroehloride for Injection （ PRI ） on persistent epdura analgesia after cesarean section. Methods The pregnant women were divided into 3 groups randomLy. Group I：PRI group（treatment group）, Group 2：morphine group （control group） and Group 3（untreated group）. Group 1 ：the 10OraL mixture included PRI 10mL＋1.6% lidocaine 12mL and NS was used under the condition of velocity=2mL/h, boost close=0.5mL/times, the whole time=15min. Group 2： the lOOmL mixture included 6mg morphine＋12.5mg promethazine＋1.6% lidocaine 12mL and NS was used under the condition of velocity=1.5mL/h.boost close=0.5mL/times,the whole time=15min. Group 3：only when women couldn＇t bear the pain of operation, 50mg pethidine＋50mg promethazine was used, and the serum prolactin was monitored before anesthesia and 24h and 48h post-operation. Results The postoperative 24h and 48h serum PRL level of Groups land 2 was obviously higher than that of Group 3（P〈0.01 ）,with a significant difference, but no significant difference between Groups 1 and 2（P〉0.05）. The side effects of Group 1 was fewer than those of Groups 2 and 3（P〈0.01）,with a significant difference. Conclusion PRI used for persistent analgesia after operation shows less effect on prolactin, conducive to its secretion.
