胸腰椎弥漫性特发性骨肥厚症的运动疗法

目的 观察运动治疗对胸腰椎弥漫性特发性骨肥厚症患者临床症状的改善作用.方法 对32例临床确诊为胸腰椎弥漫性特发性骨肥厚症患者,进行3阶段共6周的运动治疗.内容包括腰背肌群的紧张松弛训练,核心稳定能力训练和步行体能练习.所有患者在治疗前,治疗2、4、6周后,进行Oswestry腰背功能障碍指数、改良Schober试验和VAS止痛药物需要模拟指数测定.结果 治疗前后比较Oswestry指数、Schober试验值、VAS药物需要指数差异均有统计学意义（均P＜0.01）.治疗结束后,患者自身状态有明显改善21例（65.62%）,轻度改善8例（25.00%）,无改变3例（9.38%）,没有患者认为在治疗期间自身状态变得更差.结论 运动疗法能有效减轻胸腰椎弥漫性特发性骨肥厚症患者的疼痛程度,改善活动程度,提高生活质量.

Exercise therapy for patients with thoracolumbar diffuse idiopathic skeletal hyperostosis

Objective To evaluate the efficacy of exercise therapy in alleviation of symptom for patients with thoracolumbar diffuse idiopathic skeletal hyperostosis （DISH）. Methods Thirty two patients with thoracolumbar DISH received 6 weeks （3 stages） of exercise therapy, including contraction and relaxation training of lumbar dorsal muscle, core stabilization training and walking exercise. Subjects were assessed with Oswestry Disability Index, modified Schober＇s Test and Visual Analogus Scale （VAS） before and 2, 4, 6 weeks after training.Results After 6 weeks of exercise therapy, scores of all patients on Oswestry Disability Index, modified Schober＇s Test and VAS were better than before training with significant difference （P〈0.05）. Twenty one （65.62%）, 8 （25%） and 3 （9.38%） patients achieved marked, mild improvement and no alleviation of symptoms, respectively. Conclusion Exercise therapy may alleviate pain, improve functional activity and quality of life in thoracolumbar DISH patients.
