摘    要:目的 探讨甲状腺功能异常患者血糖、血脂、血尿酸水平变化及临床意义.方法 根据甲状腺功能测定结果将105例甲状腺功能异常患者分为临床甲亢组、亚临床甲减组和临床甲减组,并设健康对照组46例,同时分别测定各组空腹血糖(FPG)和血总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、血尿酸(UA)水平.结果 经年龄、性别、血压、体重等因素校正后,临床甲亢组FPG水平较对照组明显升高(P<0.01),临床甲减组较对照组降低(P<0.05),而亚临床甲减组和对照组的差异无统计学意义(P >0.05);临床甲亢组血TG、TC、LDL-C水平均较对照组明显下降(均P<0.01),临床甲减组上述各项血脂指标均较对照组明显上升(均P<0.01),亚临床甲减组上述各项指标和对照组的差异均无统计学意义(均P >0.05);3组患者的血尿酸水平均较对照组明显增高(均P<0.01). 结论甲状腺功能异常患者除血糖、血脂外,血尿酸水平亦有明显的变化,在临床中应予关注.

Abstract:Objective To investigate the variation of serum glucose, lipid and uric acid level in patients with abnormal thyroid function. Methods 105 patients with abnormal thyroid function were included and divided into three groups according to their thyroid function: clinical hyperthyroidism, clinical hypothyroidism and subclinical hypothyroidism. Their fast glucose (GLU), total cholesterol (TC), triglyceride (TG), low density lipoprotein cholesterol (LDL-C) and uric acid (UA) levels were measured and compared with 46 healthy controls. Results After adjusting for age, sex, blood pressure and body weight, we got the following results: 1. fast glucose level was higher in patients with clinical hyperthyroidism (P 〈0.01) and lower in those with clinical hypothyroidism (P 〈0.05) compared to healthy controls; patients with subclinical hypothyroidism had lower glucose level but without significant difference compared to normal controls; 2. serum lipid level, including TG, TC and LDL-C, decreased in patients with clinical hyperthyroidism (P 〈0.01) and increased in those with those with clinical hypothyroidism (P 〈0.01); patients with subclinical hypothyroidism had increased lipid level but no significant difference was found when compared to control group; 3. serum uric acid level increased in all the three groups (P 〈0.01) compared to healthy controls, but no difference was found among these three groups. Conclusion Besides serum glucose and lipid levels, physicians should also pay attention to serum uric acid level of patients with abnormal thyroid function which changes significantly.
