丙基硫氧嘧啶治疗Graves病ANCA与粒细胞减少相关性分析及激素干预研究简

目的：探讨丙基硫氧嘧啶治疗Graves病（GD）起始阶段ANCA与粒细胞减少相关性及激素干预的影响。方法：将216例初发GD患者分为单独使用丙基硫氧嘧啶A组和联合使用丙基硫氧嘧啶及地塞米松局部免疫调节法B组，测定并比较粒细胞计数及抗中性粒细胞胞浆抗体（ANCA）。结果：B治疗组粒细胞减少率低于A治疗组粒细胞减少率，B治疗组ANCA阳性率低于A治疗组ANCA阳性率，粒细胞减少率与ANCA阳性率有显著相关性。结论：地塞米松局部免疫调节法可通过抑制ANCA产生降低PTU治疗Graves病引起的粒细胞减少。

Propylthiouracil Graves in the Treatment of Disease ANCA and Granulocytopenia Correlation Analysis and Hormone Intervention Study

Objective： To observe the association between antineutrophil cytoplasmic antibody（ANCA） and neutropenia and efficacy of dehydrocortisone therapy in Graves Disease （GD） patients treated with Propylthiouracil （PTU）. Methods：We investigated 216 primary GD patients randomly divided into PTU therapy group （Group A） and combination therapy of PTU and glucocorticoid hor- mones group （Group B）. Neutrophilic granuloeyte count and ANCA level in serum were routinely measured in both groups. Re- sults： Frequency of neutropenia and ANCA positivity decreased in Group B. Conclusion ： glucocorticoid hormones therapy prevents neutropenia associated with ANCA in GD patients treated with PTU.
