摘    要:目的 探讨2型糖尿病(T2DM)并发脑梗塞与血同型半胱氨酸(Hcy)水平的关系.方法 选择50例T2DM合并脑梗塞患者纳入观察组,随机选取未合并脑梗塞的T2DM患者50例纳入对照组,比较两组患者血清Hcy、超敏C反应蛋白(hs-CRP)及颈动脉内膜中层厚度(IMT)水平,并分析血清Hcy、hs-CRP与颈动脉IMT的相关性.结果 观察组患者血清Hcy及hs-CRP均明显高于对照组,观察组患者颈动脉IMT明显高于对照组,差异均有统计学意义(均P< 0.05).2型糖尿病并发脑梗塞患者血清Hcy、hs-CRP水平与颈动脉IMT水平均呈正相关性(r=0.623、0.414,P< 0.05).结论 2型糖尿病患者脑梗塞的发生与Hcy有关,其机制可能是Hcy参与T2DM患者动脉粥样硬化进程的关系.

Abstract:Objective To investigate the relationship between serum homocysteine (Hcy) and type 2 diabetes mellitus (T2DM) complicated with cerebral infarction.Methods 50 patients with T2DM and cerebral infarction were included into observation group,and 50 patients with T2DM (not complicated with cerebral infarction) were included into control group.Serum Hcy,hs-CRP,and carotid artery intima-media thickness (IMT) were compared between two groups,and the correlation between serum Hcy,hs-CRP and carotid artery IMT were analyzed.Results The levels of serum Hcy and hs-CRP in observation group were significantly higher than those in control group,and carotid artery IMT in observation group was significantly higher than that in control group,with statistically significant differences (P<0.05).The levels of serum Hcy,hs-CRP were positively correlated with carotid artery IMT in patients with T2DM and cerebral infarction (r=0.623,0.414;P<0.05).Conclusion The occurrence of cerebral infarction in patients with type 2 diabetes mellitus is related with Hcy,and its mechanism may be that Hcy is related with atherosclerosis in T2DM.
