高龄不稳定型心绞痛合并慢性肾病患者应用不同剂量氯吡格雷的有效性及安全性

目的 分析不同剂量氯吡格雷联合阿司匹林在高龄不稳定型心绞痛合并慢性肾病患者中的治疗效果.方法 选取2013年1月至2015年12月收治的90例不稳定型心绞痛合并慢性肾病患者为观察对象,按照随机数字表法将患者分为对照组和观察组,每组45例.对照组患者给予口服氯吡格雷50mg/d+阿司匹林100 mg/d治疗,观察组患者给予口服氯吡格雷75 mg/d+阿司匹林100 mg/d治疗.连续治疗1月,对比两组疗效及药物副作用.结果 观察组疗效高于对照组(97.78％比84.44％,Z=7.717,P＜ 0.05);观察组患者药物不良反应发生率及其肾功能变化与对照组比较差异均无统计学意义(均P＞0.05).结论 75 mg/d氯吡格雷联合100 mg/d阿司匹林治疗能够有效改善不稳定型心绞痛合并慢性肾病患者的临床症状,具有安全高效和副作用少等特点,值得在不稳定型心绞痛合并慢性肾病治疗中推广应用.

Efficacy and safety of different doses of clopidogrel in treatment of elderly patients with unstable angina complicated with chronic kidney disease

Objective To analyze the effects of different doses of clopidogrel combined with aspirin in the treatment of elderly patients with unstable angina complicated with chronic kidney disease.Methods From January 2013 to December 2015,90 patients with unstable angina complicated with chronic kidney disease were selected as observation objects and divided into a control group and an observation group using random number table method,45 cases for each group.The control group were given clopidogrel,50 mg/d and aspirin,100 mg/ d and the observation group 75 mg/d and 100 mg/d for 1 month.The curative effect and drug side effects were compared between these two groups.Results The curative efficacy was higher in the observation group than in the control group (97.78％ vs.84.44％,Z=7.717,P ＜ 0.05).There were no statistical differences in the incidence of adverse drug reactions and renal function between these two groups (P ＞ 0.05).Conclusion Clopidogrel 75 mg/d and aspirin 100 mg/d in the treatment of elderly patients with unstable angina complicated with chronic kidney disease can effectively improve the patients' symptoms in patients with chronic nephropathy and is safe and efficient and has fewer side effects,so it is worth being generalized.
