HE4联合CA125在早期诊断卵巢肿瘤中的临床意义

目的 探讨人附睾蛋白4(HE4)和糖类抗原125(CA125)联合检测在卵巢肿瘤早期诊断中的应用价值.方法 回顾性分析2014年1月至2016年12月于本院就诊卵巢肿瘤患者78例,根据良恶性将患者分为卵巢癌组33例和卵巢良性肿瘤组45例,并选取正常对照组30例.对三组研究对象血清中HE4和CA125结果进行记录和比较分析.结果 卵巢癌组患者血清中HE4及CA125水平分别为(314.05±121.33) pmol/L、(474.15±148.75)U/ml,显著高于卵巢良性肿瘤组HE4(44.89±12.91) pmol/L,CA125(33.23±9.25) U/ml]与对照组HE4(30.11±6.80) pmol/L,CA125(17.71±8.93) U/ml],三组患者血清HE4和CA125差异有统计学意义(P＜0.05).HE4和CA125联合检测诊断卵巢癌的特异性为98.3％,与CA125单项检测比较差异有统计学意义(P＜0.05),与HE4单项检测比较差异无统计学意义.二者联合检测的准确度为94.4％,与CA125单项检测比较差异有统计学意义(P＜0.05),与HE4单项检测比较差异无统计学意义.二者联合检测的阳性预测值为96.3％,与CA125单项检测比较差异有统计学意义(P＜ 0.05),与HE4单项检测比较差异无统计学意义.结论 血清HE4和CA125联合检测卵巢恶性肿瘤具有互补作用,可为临床早期诊断卵巢癌提供参考依据.

Clinical significance of HE4 combined with CA125 in early diagnosis of ovarian tumor

Objective To evaluate application value of combined inspection of human epididymis protein 4 (HE4) and carbohydrate antigen 125 (CA125) in early diagnosis of ovarian tumor.Methods Retrospectively reviewed the clinical data of 78 patients with ovarian tumor in our hospital from January 2014 to December 2016,who were divided into ovarian cancer group (33 cases) and ovarian benign tumor group (45 cases) according to benign and malignant nature.And 30 cases of normal control group were selected.The levels of HE4 and CA 125 in serum of three groups were recorded and compared.Results The levels of HE4 and CA125 in serum of ovarian cancer patients were (314.05±121.33)pmol/L and (474.15±148.75)U/ml,significantly higher than those of ovarian benign tumor group (44.89±12.91)pmol/L,(33.23±9.25)U/ml] and those of control group (30.1 1±6.80)pmol/L,(17.7 l±8.93)U/ml],with statistically significant differences in the levels of serum HE4 and CA125 among the three groups (P＜0.05).The specificity of HE4 and CA125 joint detection for the diagnosis of ovarian cancer was 98.3％,with statistically significant difference compared with CA 125 single detection (P＜0.05),without statistically significant difference compared with HE4 single detection (P＞0.05).The accuracy of joint detection was 94.4％,with statistically significant difference compared with CA125 single detection (P＜0.05),without statistically significant difference compared with HE4 single detection (P＞0.05).The positive predictive value of joint detection was 96.3％,with statistically significant difference compared with CA125 single detection (P＜0.05),without statistically significant difference compared with HE4 single detection (P＞0.05).Conclusions Combined inspection of serum CA125 and HE4 were complementing each other for diagnosis of ovarian malignant tumor,which provided a reference for the early diagnosis of ovarian cancer.
