预防急诊颅脑手术术后下呼吸道感染的环节质量控制

目的 通过监控、改进急诊颅脑手术病人围手术期呼吸道管理的环节质量,预防急诊颅脑手术术后下呼吸道感染(postoperative lower respiratory tract infection,PLRTI).方法 2011-2015年,通过推行急诊颅脑手术病人围手术期呼吸道管理质量评价标准、操作规程,促进各环节质量改进;通过监测各环节合格率和同期PLRTI发生率,评价环节质量改进对预防PLRTI的效果.结果 2011年至2015年,共监测1 214例颅脑急诊手术病人,同期PLRTI发病率从36.80例/千导管日下降至5.68例/千导管日.结论 通过监控急诊颅脑手术病人围手术期呼吸道管理的环节质量可以有效降低医院内PLRTI发生率.

The targeted surveillance and intervention on procedure quality control to prevent lower respiratory tract infection after emergency craniocerebral surgery

Objective To prevent postoperative lower respiratory tract infection (PLRTI) of emergency craniocerebral surgery through surveillance and improvement on procedure quality of managing respiratory tract in all aspects during perioperative period for emergency craniotomy patients.Methods From 2011 to 2015,by means of implementing quality evaluation standard and operating procedures of managing respiratory tract during perioperative period for emergency craniotomy patients,promoted quality improvement in all aspects;through monitoring the pass rate in all procedures and the incidence of PLRTI during the same period,assessed the effect of quality improvement on PLRTI prevention.Results From 2011 to 2015,a total of 1 214 emergency craniotomy patients were under surveillance,and during the same period the incidence of PLRTI declined from 36.80 cases per 1 000 catheter days to 5.68 cases per 1 000 catheter days.Conclusion Through surveilling procedure quality of managing respiratory tract during perioperative period for emergency craniotomy patients,the incidence of PLRTI in hospitals can be effectively reduced.
