动脉阻断术联合腹腔镜腺肌瘤剥除术治疗子宫腺肌瘤的效果评价

目的 探讨采用腹腔镜下腺肌瘤剥除术联合子宫动脉阻断术治疗子宫腺肌瘤的临床效果.方法 在2014年9月至2016年2月期间本院收治的子宫腺肌瘤患者中选取84例,采用随机数字表法将其分为观察组和对照组,其中对照组患者行腹腔镜下子宫腺肌瘤剥除术,观察组患者则采用腹腔镜下子宫腺肌瘤剥除术联合子宫动脉阻断术进行治疗,对两组患者的手术情况及其术后复发情况进行观察分析.结果 观察组患者术中出血量(89.5±17.6) ml]、术后排气时间(19.6±5.7)h]均明显优于对照组(154.7±25.1) ml,(25.1±7.3) h] (P＜0.05);观察组患者术后痛经程度评分(2.8±0.9)]、子宫体积缩小幅度(46.9±5.5)％]及复发率(2.38％)均明显优于对照组(3.6±1.2),(35.1±7.4)％,21.43％],差异有统计学意义(P＜0.05).结论 采用腹腔镜下腺肌瘤剥除术联合子宫动脉阻断术治疗子宫腺肌瘤患者,有助于彻底清除残留腺肌瘤组织,防止复发,并可对术中出血进行有效控制,对于患者术后及早恢复具有十分积极的意义.

Effect evaluation of uterine artery blockage combined with laparoscopic adenomyoma stripping in the treatment of endometrioma

Objective To explore the clinical effect of uterine artery blockage combined with laparoscopic adenomyoma stripping in the treatment of endometrioma.Methods 84 cases of endometrioma admitted in our hospital between September 2014 and February 2016 were randomly divided into observation group and control group.The control group underwent laparoscopic adenomyoma stripping,while the observation group underwent uterine artery blockage combined with laparoscopic adenomyoma stripping.The operation and recurrence of the two groups were observed and analyzed.Results The intraoperative blood loss,postoperative evacuation time in the observation group were significantly better than those in the control group (89.5± 17.6)mi vs.(154.7±25.1)ml,(19.6±5.7)h vs.(25.1±7.3)h;P＜0.05].The postoperative dysmenorrhea score,uterine volume reduction,and recurrence rate in the observation group were significantly better than those in the control group (2.8±0.9) vs.(3.6±1.2),(46.9±5.5)％ vs.(35.1±7.4)％,2.38％ vs.21.43％;P＜0.05].Conclusions Uterine artery blockage combined with iaparoscopic adenomyoma stripping in the treatment of endometrioma,contributes to the complete removal of residual adenomyoma tissue to prevent the recurrence,and effective control of intraoperative bleeding,has very positive significance in the early recovery of the patients after surgery.
