持续质量改进降低农村腹膜透析患者腹膜炎发生率的效果评价

目的 评价持续质量改进(continuous quality improvement,CQI)管理工具在降低农村腹膜透析患者腹膜炎发生率的有效性.方法 回顾性分析2012年1月至2013年12月本中心农村患者腹膜炎患者的临床资料,发现不重视无菌操作是诱发腹膜炎的主要原因,针对此运用计划、实施、检查和修正这个循环不止的科学程序进行持续质量改进,针对腹膜炎发生的高危因素和工作流程进行改进,并比较实施前后腹膜炎发生率.结果 本中心有腹膜透析随访患者136例,总体腹膜炎发生率是1次/19.14病人月,其中农村患者发生率是1次/17.45病人月.实施持续质量改进后,本中心农村患者腹膜炎发生率由1次/17.45病人月降至1次/31.45病人月.结论 科学的持续质量改进管理方法可以有效降低农村腹膜透析患者腹膜炎发生率.

Continuous quality improvement in reducing incidence of peritonitis in rural patients undertaking peritoneal dialysis

Objective To evaluate the effect of continuous quality improvement in reducing the incidence of peritonitis in rural patients undertaking peritoneal dialysis.Methods The clinical data of rural peritonitis patients treated at our center from January,2012 to December,2013 were retrospectively analyzed and found that paying no attention on aseptic operation was a major cause of peritonitis;and correspondingly,planning,implementation,checking,and correcting the scientific program for continuous quality improvement were taken and measures for peritonitis risk factors and working process were improved.The incidences of peritonitis were compared between before and after the implementation.Results 136 patients undertaking peritoneal dialysis at our center were followed up.The total incidence of peritonitis was 1 time / 19.14 patients month;the incidence of peritonitis of rural patients was 1 time / 17.45 patients month.After the implementation of continuous quality improvement,the incidence of peritonitis of rural patients at our center decreased from 1 time/17.45 patients month to 1 time/31.45 patients month.Conclusion Continuous quality improvement of scientific management method can effectively reduce the incidence of peritonitis in rural patients undertaking peritoneal dialysis.
