脑室-腹腔分流术后颅内感染的相关因素及对策

目的探讨脑室．腹腔分流术（V-Ps）后颅内感染的相关危险因素及对策。方法回顾性分析430例施行了分流手术的病例资料，利用Logistic回归分析9项可能相关指标对V—Ps术后感染的影响。结果Logistic回归分析显示感染与术后脑脊液漏、激素和预防性抗生素的应用、手术条件有关。结论本结果有助于预见分流术后感染的危险性，从而减少V-Ps术后感染的发生。抗生素应用基础上移除分流系统，待感染完全控制后再植入新的分流系统是有效的治疗方案。

An analysis on the risk factors related to the infection after opertion of V-Ps

Objective To analyze the relevance of risk factors and preventive strategy of infections after V-Ps. Methods 430 patients with intracranial-infections after V-Ps were retrospectively reviewed to determined the effects of 9 related factors. Results Of the 430 patients, infections were observed in 30 cases （6.98 % ）. Logistic regression analysis showed that the incidence of infection was related to the postoperative leak of CSF, the use of preventive antibiotic and hormore, the operation condition （OR = 52. 311,0. 260,3. 325,0. 247 ）. Conclusion The results of this study can help in identifying patients with a high risk of infection after V-Ps. The removal of an infected shunt which based on the usage of antibiotic,followed by installation of a new shunt when CSF sterility is achieved,is the effective and reliable method of treatment for CSF shunt infection.
