2型糖尿病合并冠心病患者血浆MCP-1含量的变化

摘    要:目的:观察2型糖尿病大血管病变患者血浆MCP-1含量变化。方法:选择176例2型糖尿病患者,其中糖尿病合并冠心病组(DM+CHD组)75例,糖尿病非合并冠心病组(DM)101例,另外选择70例健康者作为对照组。应用Sandwich ELISA方法测定血浆MCP-1含量变化。结果:DM组、DM+CHD组血浆MCP-1含量均显著增高,DM+CHD组血浆MCP-1含量较DM组含量增高更加明显,HDL-C、SBP、FBG、HbAlc与MCP-1水平显著相关,反映冠脉狭窄程度的Gensini积分与MCP-1含量显著正相关,多因素logistic回归分析表明,MCP-1是糖尿病合并冠心病发生的危险因素。结论:糖尿病患者血浆MCP-1含量显著增高,合并冠心病患者MCP-1含量增加更加显著,MCP-1含量与冠脉狭窄程度显著正相关,是糖尿病患者发生冠心病的独立危险因素。

Abstract:Objective:To investigate the change of plasma MCP-1 in type 2 diabetic patients with coronary heart disease. Methods: One hundred and seventy-six patients with type 2 diabetes mellitus were chosen and divided into 2 groups: DM group (101cases) and DM + CHD group (75cases). In addition, 70 healthy people were chosen as control group. The level of plasma MCP-1 was measured by the method of sandwich ELISA. Results: Compared with that in control group, the level of MCP-1 was significantly increased in both DM + CHD group and DM group. Level of MCP-1 was higher in DM + CHD group than that in DM group. The MCP-1 content was significantly correlated with the levels of SBP, HDL-C, HbAlc, FBG. The Gensini score which reflects the degree of severity of coronary artery narrowing was significantly correlated with the level of MCP-1, and multiple logistic regression analysis showed that MCP-1 was an independent risk factor of CHD in diabetic patients. Conclusion:The level of MCP-1 is significantly increased in diabetic patients and correlated with degree of the coronary artery narrowing, and moreover, is an independent risk factor for type 2 diabetes mellitus patients with CHD.
