阴式子宫肌瘤剔除术18例临床分析

目的探讨阴式子宫肌瘤剔除术（TVM）的手术技巧及治疗效果。方法回顾分析安徽省六安市中医院妇产科施行阴式子宫肌瘤剔除术18例手术方式及治疗效果，并与同期腹式子宫肌瘤剔除术18例比较。结果18例子宫肌瘤均成功采用阴式剔除。平均手术时间、术中平均出血量2组比较差异无显著性意义（P〉0．05）。阴式组术后排气时间、术后疼痛程度及术后住院天数等均较腹式组短（P〈0．05）。结论阴式子宫肌瘤剔除具有创伤小、恢复快等优点，是一种安全有效保留子宫的微创手术方法。

Clinical Analysis of Transvaginal Myomectomy

Objective To evaluate the surgical techniques, therapeutic effect of transvaginal myomectomy. Methods 18 cases treated by transvaginal myomectomy were retrospectively analyzed as the study group, which was selected from February 2006 to July 2009, and 18 cases treated by celiomyomectomy during the same period as the control group. Results Transvaginal myomectomy was successfully done for the patients in study group. There were no significant differences for the mean operating time, volume of blood loss during operation between two groups （P 〉 0.05 ） ; the mean exsufflation time and postoperative hospitalization days in the study group were significantly shorter than that in the control group（P 〈 0.05）. Conclusion Transvaginal myomectomy is a safe and effective minimally invasive surgery method. Comparing with the celiomyomectomy, the wound in transvaginal myomectomy operation is lighter, its recovering time is shorter and it needs no expensive equipments. However, celiomyomectomy has broader indications, and can reject more hysteromyoma and larger ones.
