成人盘状软骨损伤的关节镜下诊治

目的探讨成人盘状软骨损伤的特点及关节镜下诊治方法及其疗效。方法自2004年8月至2006年10月，38例（40膝）外侧盘状软骨损伤的患者分别采用关节镜下成形术12例，次全切术28例。其中2例患者双膝外侧盘状软骨损伤分两次手术行次全切除术。结果34例（36膝）获得17．1个月（5～57个月）的随访。按Ikeuchi法评定疗效：优26例（72．2％），良8例（22．2％），可3例（8．3％）。结论关节镜下治疗盘状软骨损伤，关节侵扰少，恢复快。成人症状性盘状软骨，无论破裂与否均应早期行关节镜下手术治疗。

Arthroscopic diagnosis and treatment for the discoid lesion in adults

Objective To explore the peculiarity of the discoid lesion in adults and the effective way of diagnosis and treatment with arthroscopie technique. Methods From Aug 2004 to Oct 2006, a series of 38 patients (40 knees)who had undergone the arthroscopic procedure for the problem of discoid. Among them, discoidplasty operation were performed in 12 cases,total meniscectomy were performed in 28 cases,2 eases(4 knees) underwent separate operations of total meniscectomy for the bilateral discoid lesion. Results 36 knees in 34 cases were followed up at least 17.1 months(5～57 months). To evaluate the outcome according to the Ikeuchi's standard, excellent and good in 94.4%. Conclusion To deal with the lesion of discoid in adults by the arthroscopic technique, the advantage are less invasive injury to the knee joint and shorten the period of rehabilitation. Once the diagnosis of discoid in adults were identified, whatever the discoid tear or not, that should be operated by the arthroscopic technique.
