标准大骨瓣减压意义

目的探讨标准大骨瓣在重型颅脑损伤救治中的减压意义,加深临床对标准大骨瓣减压作用的理解。方法结合我院2000年1月-2007年1月46例重型颅脑损伤的患者资料进行回顾性分析。结果本组46例患者均行标准大骨瓣减压,术中全部放置颅内压监测探头,术后持续颅内压监测1周,联用人血清蛋白或血浆、甘露醇、速尿脱水,颅内压一般在15-30 mmHg范围内。术后3d、1周、2周、1个月复查CT。2例发生右额叶脑梗死,1例发生右顶枕叶脑梗死。术后随访6个月至1年,效果良好26例,中残8例,重残6例,植物生存4例,死亡2例。结论标准大骨瓣入路可多途径的达到减压作用,在重型颅脑损仍的救治中有着重要的内、外减压意义。
