| 摘 要： | 目的研究白细胞介素及选择素在小儿急性细菌性脑膜炎及病毒性脑膜炎的临床诊断、治疗、病情转归和判断预后的应用价值。方法以ELISA法分别检测小儿急性细菌性脑膜炎及病毒性脑膜炎患者治疗前后以及正常对照组脑脊液白细胞介素1(Interleukine-l;IL-1);用流式细胞仪测定选择素(CD62L、CD62P、CD62E)含量。并探讨各指标与颅内感染鉴别诊断与预后的关系。结果小儿急性细菌性脑膜炎及病毒性脑膜炎患者治疗前脑脊液IL-1;CD62P、CD62E选择素含量较对照组明显升高(p<0.01),而L-选择素(CD62L)则无显著变化(p>0.05);急性细菌性脑膜炎及病毒性脑膜炎患者治疗后以上各指标明显下降(p>0.05)。结论脑脊液白细胞介素1(IL-1);选择素(CD62L、CD62P、CD62E)含量选择素水平的变化与颅内感染的临床变化有较密切的关系,可以作为大脑炎症病情变化的监测指标和预后判断指标,同时也为在脑炎发病早期使用抗粘附分子抗体及药物治疗提供临床实验依据。
