后腹腔镜途径治疗肾盂输尿管连接部狭窄体会

目的分析后腹腔镜途径治疗肾盂输尿管狭窄的疗效,探讨该术式的适应症和技术要点。方法回顾分析7例肾盂输尿管连接部狭窄患者的临床资料,所有病例均行后腹腔镜下肾盂成形术,术中均留置双J管。结果7例手术全部成功,术中无并发症发生。平均手术时间为260min(195～370min);平均出血量70ml(50ml～190ml);所有患者术后5～9天出院。术后随访5～10个月,B超示积水减轻,症状缓解。结论后腹腔镜下肾盂成形术治疗肾盂输尿管连接部狭窄安全、有效,但需要一定的腹腔镜外科训练。

Treatment of ureteropelvic junction obstruction by Retroperitoneal laparoscopic pyeloplasty

Objective To analyse the surgical outcome and explore the indication and major techniques by retroperitoneal laparoscopic pyeloplasty for ureteropelvic junction obstruction.Methods Review the clinical data of 7 cases with ureteropelvic junction(UPJ)obstruction.All patients underwent laparoscopic pyeloplasty.Double J stents were used in all cases.Results All the procedures were successfully completed.The mean operation time was 260 min(range from 195 to 370);The mean blood loss was 70ml(range from 50 to 190).The hospital stay was 5 days to 9 days.Ultrasound B investigation showed hydronephrosis reduced and symptom relieved in all cases during 5 months to 10 months follow-up.Conclusion Laparoscopic pyeloplasty is safe and effective for treatment of ureteropelvic junction obstruction.However,it requires laparoscopic training.
