三维超声重建冠状断面检查诊断乳腺良恶性肿物的探讨

摘    要:目的 探讨三维超声重建冠状断面在乳腺良恶性肿物鉴别诊断中的应用价值。方法  89例经病理证实的乳腺实性肿物 ,其中恶性组 3 6例 ,良性组 5 3例。先用二维超声观察肿物的形态、边缘及回声等特征 ,再用三维超声重建冠状断面观察肿物边缘及和周围组织的关系 ,分析三维超声重建冠状断面上各种征象的诊断价值。结果 重建冠状断面上 ,完整界面回声诊断乳腺良性肿物的特异性为 94.4% ,敏感性为 64 .2 %。汇聚征诊断恶性肿物的特异性为 93 .4% ,敏感性为 5 2 .8%。结论 三维重建冠状断面检查可为超声鉴别乳腺肿物的良恶性提供有价值的新指标 ,是二维超声的良好补充。

Abstract:Objective To evaluate the significance of the coronal plane reformatted with three dimensional ultrasound (3D) in distinguishing malignant from benign masses of breast. Methods Eighty nine solid masses of breast were divided into two groups according to histopathologic results and examined with two dimensional ultrasound (2D) firstly, then with the 3D ultrasound. Some sonographic features of the coronal plane of the masses were studied and the significance of the features was evaluated. Results The specificity and sensitivity of compressing pattern in the coronal plane to indicate benign masses were 94.3% and 64.2% respectively, and those of the converging pattern to indicate malignant masses were 93.3% and 52.8% respectively. Conclusion Coronal plane reformatted with 3D ultrasound is useful in diagnosis of breast masses and is compentary to 2D ultrasound.
