山东省农村居民伤害流行病学及其经济负担研究

目的]了解山东省农村居民伤害的发生率、死亡率,探讨其流行特征,估算伤害导致的潜在寿命损失年数(YPLL)及其经济负担。方法]按照多阶段分层随机抽样方法,根据经济水平随机抽取6个县(市)的14490名农村常住居民,采用横断面调查的方法,由经过统一培训的调查员入户调查全部家庭成员2003年伤害事件发生情况。结果]2003年山东省农村常住居民伤害事件发生率为8.45%,标化伤害事件发生率为8.50%。伤害事件发生率男性高于女性;5～14岁年龄组伤害事件发生率最高,其次为60岁以上的人群;伤害事件发生率的前3位分别是跌倒/坠落(1.58%)、交通事故(1.44%)、扭伤(1.17%)。平均每例伤害死者YPLL为33.00年,WYPLL为27.30年,VYPLL为15.20年;共发生1225次伤害事件,导致的经济损失为230.60万元,平均每个伤害事件(含死亡)造成的经济损失为1882.45元。结论]山东省农村常住居民伤害事件发生率较高,对居民及其家庭造成了较大的经济负担。应加强对重点人群的健康教育和干预。

STUDY ON THE EPIDEMIOLOGICAL SURVEY AND ECONOMIC BURDEN OF INJURIES AMONG RURAL RESIDENTS IN SHANDONG PROVINCE

Objective]To comprehend the incidence,mortality and epidemic characteristics of injury and to estimate the years of potential life lost(YPLL)and economic burden due to injuries among rural residents in Shandong province.Methods]By multi-stage stratified sampling,six counties(14,490 inhabitants included)in different economic levels were randomly selected.With household questionnaires about injury events during 2003,a cross-sectional study was conducted by unified-trained inquirers among all the family members.Results]The injury incidence during 2003 was 8.45% in some rural areas in Shandong province and the standardized incidence was 8.50%.The incidence rate of male was higher than that of female.The group of 5-14-year-old had the highest injury rate,the group of over 60-year-old ranked secondly.The three leading causes of injury were falling/dropping(1.58%),traffic accidents(1.44%)and strain(1.17%).YPLL attributed to average injury death was 33.00,work years of potential life lost(WYPLL)was 27.30 and valued years of potential life lost(VYPLL)was 15.20.A total of 1 225 injury happened during the year,which led to economic loss of 2 306 000 RMB yuan.The average economic loss for each injury case(including injury death)was 1 882.45 RMB yuan.Conclusion]The injury incidence rate is higher among rural constant residents in Shandong province,and injury events bring heavy economic burden to individuals and their families.Health education and intervention should be strengthened to key population.
