130例多发性骨髓瘤临床特点与细胞形态学分析

目的:探讨多发性骨髓瘤(MM)的主要临床特点以及骨髓细胞形态学在MM诊断中的作用。方法:回顾分析130例MM患者临床特点并分析其骨髓及外周血涂片,观察骨髓瘤细胞的比例和形态特征。结果:130例MM患者临床表现中主要有贫血99例(76.2%)、骨痛81例(62.3%)、骨质破坏83例(63.8%)、肾功能损害70例(53.8%)、出血6例(4.6%)等。骨髓涂片中骨髓瘤细胞比例≥10%的126例(96.9%),骨髓瘤细胞比例在6.0%～10.0%的3例(2.3%),骨髓瘤细胞比例不增高的1例(0.7%)。结论:MM的诊断中,骨髓细胞形态学是非常重要的方法,但同时需结合临床表现及影像学、生化等方法。

Analysis of Clinical Features and Cell Morphology in 130 Patients with Multiple Myeloma

Objective To investigate the clinical features in patients with multiple myeloma(MM)and the significance of bone marrow cell morphology in MM diagnosis.Methods The clinical features of 130 patients with MM were retrospectively analyzed and further analyzed myeloma cell ratio and morphology type in bone marrow or peripheral blood smear slide.Results The clinical features in 130 patients with MM are anemia(99/130,76.2%),osteodynia(81/130,63.8%),osteoclasia(83/130,63.8%),renal functional abnormality(70/130,53.8%),bleeding(6/130,4.6%).The patients with myeloma cell percentage>10% in bone marrow slide were 126/130(96.9),patients with myeloma cell percentage between 6%～10% are 3(2.3).Conclusion Morphology of bone marrow cell is an important method in MM diagnosis,while the clinical features,imageology and biochemistry test also should be recommended.
