老年心力衰竭患者血清胆红素水平的临床研究

目的：探讨老年心力衰竭患者血清胆红素水平的临床意义。方法：将52例老年心力衰竭患者纳入该研究,并选择35例健康老年人作为对照,比较心衰组与对照组以及心衰各亚组间血清胆红素水平。结果：心衰组胆红素水平（15.84±3.16）μmol/L]明显高于健康组（8.48±2.28）μmol/L]（P〈0.05）,心功能Ⅲ～Ⅳ级组胆红素水平（18.05±3.72）μmol/L]明显高于心功能Ⅱ级组（12.11±2.81）μmol/L]（P〈0.05）,心衰发作高峰组胆红素水平（15.84±3.16）μmol/L]明显高于心衰症状缓解组（11.48±3.04）μmol/L]（P〈0.05）。随访3个月病情稳定组血清胆红素水平高于反复加重组（P〈0.05）。结论：老年心衰患者血清胆红素水平增加、提示血红素氧化酶/一氧化碳、胆红素系统参与心力衰竭病生过程,启动机体内源性保护机制。

The clinical significance of the serum bilirubin level in gerontal patients with congestive heart failure

Objective： To investigate clinical significance of serum bilirubin level in gerontal patients with congestive heart failure and to explore the relationship between CHF and the heme oxygenase,carbon monoxide,bilirubin system.Methods： 52 cases of patients with CHF were collected,the patients were divided into level Ⅱ group and level Ⅲ-Ⅳgroup.And 35 cases of health people as the control（health group）,the bilirubin level in the CHF group（consist of subunit groups） and health group were compared.Results： The level of serum bilirubin in the CHF patients were（15.84±3.16） μmol/L,and were significantly more increased than those in healthy group（8.48±2.28） μmol/L（P0.05）.The level of serum bilirubin in level Ⅲ-Ⅳ group were（18.05±3.72） μmol/L,and were significantly more increased than those of level Ⅱgroup（12.11±2.81） μmol/L（P0.05）.The level of serum bilirubin in the onset patients were（15.84±3.16） μmol/L were significantly more increased than those of catabasis patients（11.48±3.04） μmol/L（P0.05）.Conclusion： There is a higher expression of the serum bilirubin in patients with CHF,due to the properties of HO and its products（CO）,we hypothesize that HO would play an important role in the regulation of the CHF function.
