| 摘 要： | 目的：评价三维彩色血管能量成像（3D—CPA）在鉴别卵巢肿物良恶性方面的临床应用价值。方法：对2010年2～9月我院收治的43例卵巢肿物患者，于术前行三维彩色多普勒超声检查，记录血流动力学参数．即血管形成指数（vascularization index，VI）、血流指数（flow index，FI）、血管形成-血流指数（vascularization—flow index，VFI），并对血流丰富对应区域组织标本进行血流分型。结果：依据病理结果，恶性病例为19例。良性病例为24例．卵巢恶性肿物组的VI、FI、VFI参数值均显著高于卵巢良性肿物组，差异有统计学意义（P〈0．05）；卵巢恶性肿物组以Ⅱ、Ⅲ级血流为主，良性肿物组多无血流或以I级血流为主，两组之间差异有统计学意义（P〈0．05）；三维超声诊断符合率为94．7％（18／19），二维超声诊断符合率为83．3％（20／24）；HI型的血管参数VI、FI、VFI值显著高于Ⅱ型相应的血管参数值，两组之间差异有统计学意义（P〈0．05）；卵巢良性肿物组血流的检出率为75％（18／24），恶性肿物组血流的检出率为100％（19／19）。结论：3D—CPA可以客观地评价卵巢肿瘤的血管，对卵巢肿瘤的良恶性鉴别具有重要的参考意义。
