后路椎弓根钉内固定植骨治疗胸腰椎骨折

目的总结应用后路椎弓根钉系统内固定植骨治疗胸腰椎骨折的临床效果。方法采用后路椎弓根钉内固定系统加后路横突椎板间植骨或伤椎经椎弓根椎体内植骨治疗胸腰骨折48例。结果经9～24个月的随访,48例椎体前缘高度由术前的平均33%恢复到术后的94%,Cob角由手术前的平均24°降低到手术后的平均4°,椎管内骨块复位率达91%。结论后路椎弓根钉内固定加植骨术治疗胸腰椎骨折能获得良好的骨折复位固定脊柱及椎管减压的目的,后期随访椎体高度及Cob角丢失率低,是治疗胸腰椎骨折的一种简单有效的方法。

Posterior Pedicle Screw Internal Fixation in Treating Thoracolumbar Vertebral Fracture

Objective To study the clinical outcomes of posterior pedicle screw internal fixation with bone grafting in treating thoracolumbar fracture.Methods Forty-eight patients underwent posterior pedicle screw internal fixation with posterior transverse process-lamina or pedicle-body bone grafting for the treatment of thoracolumbar fractures.Results After 9-to 24-month follow-up,the mean anterior margin height of vertebral bodies was increased from 33% preoperatively to 94% postoperatively.The Cobb′s angle was reduced from 24° preoperatively to 4° postoperatively.The reduction rate of intraspinal bone mass was 91%.Conclusion The posterior pedicle screw internal fixation with bone grafting results in a good fracture fixation,stabilization of spinal column,and spinal decompression,showing a satisfactory follow-up vertebral body height and Cobb＇s angle,as a simple and effective treatment of thoracolumbar fracture.
