食管癌贲门癌患者术后心律失常187例临床分析

目的 探讨食管癌贲门癌患者术后心律失常的发生原因及预防措施。方法 对625例食管癌责门癌患者的临床资料进行回顾性分析。结果术后共发生心律失常187例。＜60岁者术后心律失常的发生率为24．6％，≥60岁者为36．6％；术前合并心血管疾病者术后心律失常发生率为87．2％；食管癌患者术后心律失常发生率为37．7％，贲门癌为22．1％；颈部吻合者术后心律失常发生率为33．0％，弓下吻合者为22．3％；各组间均有显著性差异。结论 年龄、术前合并心血管疾病、吻合部位与术后心律失常的发生密切相关。充分的术前准备，加强术后监护，纠正缺氧可以预防或减少心律失常的发生。

Clinical Analysis of 187 Postoperative Cases of Esophageal and Cardiac Carcinoma with Arrhythmia

Objective To determine the causes of arrhythmia in postoperative patients with esophageal and cardiac carcinoma,and the way to prevent it.Methods The clinical data of 625 cases were analyzed retrospectively.Results There were 187 patients with arrhythmia during the postoperative period.The arrhythmia incidence in patients under and above 60 years old were 24.6% and 36.6% respectively.The arrhythmia incidence in patients with cardiovascular disease before operation was 87.2% .The arrhythmia incidence in patients with esophageal carcinoma and cardiac carcinoma was 37.7% and 22.1% respectively.The arrhythmia incidence in patients with anastomosis in neck and under aortic arch was 33.0% and 22.3% respectively.There were significant differences in these groups.Conclusion These results suggest that age,preoperativly existed cardiovascular disease,anastomotic location are correlated with arrhythmia.Sufficient preoperative management,intensive postoperative monitor and hypoxemia correction can prevent arrhythmia or decrease the incidence.
