重症胰腺炎94例综合治疗体会

目的总结重症胰腺炎临床治疗效果。方法资料来源于1996年1月～2005年6月本科收治94例重症胰腺炎,在治疗上分两个阶段,第一阶段1996年1月～1997年12月以手术治疗为主(手术组)26例;第二阶段1998年1月～2005年6月为保守组以综合治疗配合CT动态观察下非手术为主68例。结果手术组:治愈22例,死亡4例,治愈率84.6%;保守组:治愈64例,死亡4例,治愈率94.1%。第二阶段有2例诊断暴发性胰腺炎,手术治疗无效死亡。结论早期手术能降低病死率。以非手术为主综合治疗措施能提高重症胰腺炎治疗效果;提高治疗暴发性胰腺炎的疗效是的今后的研究方向。

Experience of combined treatment for Severe Pancreatitis(A report of 94 cases)

Objective To summarize the effectivity of clinical tretment for severe pancreatits. Methods From Jan. 1996 of Jun 2005 94 cases with severve pancreatits were treated.These cases were divided into two stages. 26 patients received operation from Jan. 1996 to Dec. 1997 were in the first stage (operative group). 68 patients received combined treatment and dynamic CT observation from Jan. 1998 to Jun. 2005. were in second stage (conservative group). Results 22 patients were cured and 4 died in operative group. The survivl rate was 84.6%.64 patients were cuned and 4died in conservative group.Two cases with dide from fulminant pancreatitis in second stage.Survial rate 94.1%.Conclusion Early operation can,t decrease the mortality of severe pancreatitis. Combined conservative treament can improve the curative effect of severe pancreatitis and may be the best way to treat severe pancreatitis at present. For impoving the curative effect of fulminant pancreatitis,it is reseach direction in the future.
