腹腔镜与小切口胆囊切除术的对照研究

目的探讨腹腔镜与小切口胆囊切除术的利弊。方法将90例胆囊结石、急性胆囊炎和胆囊息肉患者随机分为腹腔镜胆囊切除组（LC）和小切口胆囊切除组（MC），各45例。对比分析其手术切口长度、手术时间、术中出血量、术后进食时间、术后下床活动时间、住院时间、输液时间、医疗费用。结果腹腔镜和小切口胆囊切除术皆有创伤小、恢复早的特点，两者术后恢复差异无显著性。但腹腔镜胆囊切除组医疗费用高于小切口胆囊切除组，差异有显著性。结论两种术式各有优缺点，其中一种术式均不适应所有胆囊切除适应证的患者。选择何种术式，应根据患者的具体情况、设备条件和术者的经验来决定，

Comparitive Study of Laparoscopic Cholecystectomy and Mini-incision Cholecystectomy

Objective To analyze the advantages and disadvantages of laparoseope cholecystectomy and mini-incision cholecystectomy. Methods 90 patients with gallbladder calculus or acute and chronic cholecystifis or gallbladder polyp were randomly divided into the laparoscopic cholecysteetomy（LC） group and mini-incision the eholeeysteetomy（MC） group,each 45 examples. Contrasts in its incision lengths, duration （time） of operation, amount of intraoperative bleeding, time of gastrointestinal function recovery, intravenous infusion, time and expense of hospitalization. Results Both of laparoscope and mini-incision eholecystectomy had the features of less injury and early recovery. There was no significant difference in the post-operative recover）, between the two groups. The medical expenses of the patients with laparoseopic cholecysteetomy were higher than those of the patients with mini-incision cholecystectomy. Conclusion There is no significant overall advantages of one method over the other. An appropriate choice for the method of ebolecystectomy should be made based on the condition of patient,instrument and the experience of surgeon.
