创伤合并内毒素诱发弥散性血管内凝血模型的建立

摘    要:目的:建立一种创伤后感染继发弥散性血管内凝血(DIC)模型,观察其病理变化,为临床防治DIC提供依据。方法:将家兔随机分为正常对照组(C组)、单纯创伤组(T组)、内毒素组(E组)、创伤合并内毒素组(T+E组)。在造模1h和造模后2、6、12h取血,检测PT、APTT、PLT、D-二聚体等指标,观察家兔精神等一般情况变化及取家兔肺、肾脏进行组织病理学观察。结果:T组、E组、T+E组PLT、APTT、PT、D-二聚体在不同时相,与C组比较差异有统计学意义(P<0.05或P<0.01)。与T组相比,T+E组6h时PT、APTT、D-二聚体均明显延长或升高,PLT明显降低,差异有统计学意义(P<0.05或P<0.01)。与E组相比,T+E组12hPT、APTT明显延长,差异有统计学意义(P<0.05或P<0.01);PLT与D-二聚体从6h起差异有统计学意义(P<0.05或P<0.01)。T组、E组、T+E组肺肾病理均可见微血栓形成,T+E组尤甚。结论:创伤联合内毒素可制备DIC兔模型,反映创伤后继发感染致DIC发生的病理过程,并且该模型各项指标较单纯创伤或感染引起的DIC更有意义。
