异位妊娠危险因素Logistic回归分析

目的研究异位妊娠发病的相关因素。方法采用回顾性的病例-对照研究方法,使用“异位妊娠调查登记表”对419名异位妊娠及386名宫内妊娠(早、中孕引产、妊娠恶阻)进行异位妊娠发病相关因素的多元回归分析。结果病例组在盆腔炎史、输卵管手术史、剖宫产史、生育史、人流史、IUD方面与对照组有显著差异。而在月经不调史、痛经史、阑尾炎手术史、自然流产史、不孕史方面无显著性差异。结论异位妊娠的发病与盆腔炎史、输卵管手术史、剖宫产史、生育史、人流史等因素有关,中国传统节气在异位妊娠的发病中可能起到某种作用,小暑节气可能是发病高峰。

Logistic Analysis on the Risk Factors of Ectopic Pregnancy

Objective To study the risk factors for ectopic pregnancy. Methods Making use of theregister of ectopic pregnancy and intra-uterine pregnancy, carry out a case-control study including 419 ectopicpregnancy and 386 intra-uterine pregnancy, multi-factorial analysis were fitted by non-conditional logistic regressionmodel. Results The main risk factors for ectopic pregnancy were tubal surgery with the odds ratio (OR)=4.17,95% confidence interval(CI):2.02~8.60 and the use of intra-uterine device (IUD) with OR=9.87, 95% CI:1.19~81.63.The other risk factors were previous pelvic infections history with OR=1.89, 95% CI:1.01~3.55; previous induced abortionswith OR=1.50, 95% CI:1.30~1.73; Obstetric histories with OR=2.86, 95% CI:2.18~3.76 and caesarean section withOR=2.41, 95% CI:1.02~5.68. Conclusion The main risk factors were tubal surgery and the use of intra-uterinedevice. The previous pelvic infections history, previous induced abortions, obstetric histories and caesarean sectionwere associated with a risk of ectopic pregnancy.
