低位三管引流在直肠癌保肛术后预防吻合口瘘中的应用及护理

目的:探讨低位三管引流在直肠癌保肛术后预防吻合口瘘的临床价值及护理。方法:将我院收治并行保肛手术的168例直肠癌患者分为两组,观察组90例术中给予低位三管引流(2根经肛肠腔内置于吻合口上端、下端,1根经肛旁置于骶前腔);对照组78例采用传统引流(经肛旁放置一骶前腔引流管)。观察两组术后切口感染及吻合口瘘的发生率。结果:对照组切口感染发生率为7.77%(7/78),观察组为为8.89%(8/90),两组比较无统计学差异(P>0.05);对照组吻合口瘘发生率为11.54%(9/78),观察组为3.33%(3/90),观察组明显低于对照组,差异有统计学意义(P<0.05)。结论应用低位三管引流能够显著降低直肠癌患者保肛术后并发吻合口瘘的发生率。

Nursing of low three drainage used in rectal cancer patients to prevent clinical anastomotic leakage.

Objective： To study the low three drainage in rectal cancer prevention of anastomotic leakage after the clinical value of its care.Methods： In parallel in our hospital 168 cases of sphincter preserving surgery for rectal cancer were divided into two groups,patients in the observation group of 90 patients given low three drainage（two placed by the endovascular anastomosis anorectal top,bottom,one afterPerianal placed presacral cavity）;the control group 78 patients with traditional drainage（anal cavity placed next to a presacral drainage）.Postoperative wound infection were observed and the incidence of anastomotic leakage.Results： In the control group wound infection rate was 7.77%（7/78）,the observation group was 8.89%（8/90）,showed no significant difference（P0.05）;control group,the incidence of anastomotic leakage 11.54%（9/78）,the observation group was 3.33%（3/90）,the observation group was significantly lower than the control group,the difference was statistically significant（P0.05）.Conclusion The low three drainage can significantly reduce the rectal sphincter preservation in patients with postoperative incidence of anastomotic leakage.
