足踝部软组织腱鞘囊肿的MRI诊断

摘    要:目的 探讨足踝部腱鞘囊肿的临床和MRI 特点.方法 对12 例足踝部软组织腱鞘囊肿患者的临床和MRI 资料进行了回顾性分析.患者男女比例为1∶5,平均年龄47 岁.结果 12 例病变中2 例位于第一跖趾关节附近,4 例位于足背内侧部,5 例位于踝部,1 例位于足跟部.与肌肉相比所有病变在T1WI 都呈均匀稍低信号,在T2WI呈明显高信号,增强扫描表现为薄壁强化;10 例病变呈多房表现;5 例病灶周围可见轻度水肿.结论 足踝部腱鞘囊肿常见于中年女性,多位于踝部和足背内侧部;常呈多房样表现,与肌肉相比在T1WI 呈稍低信号,在T2WI 呈高信号,增强扫描表现为薄壁强化.

Abstract:Objective To explore the clinical and MR imaging features of soft tissue ganglion cyst in the foot and ankle. Methods Clinical and MR imaging data of 12 patients (male to female ratio 1:5, mean age 47 years) with soft tissue ganglion cysts in the feet and ankles were retrospectively analyzed. Results The 12 ganglion cysts were located near the first metatarsophalangeal joint (2), in the medial dorsum of foot (4), in the ankle (5) and in the heel (1). Compared with muscle, all lesions showed homogeneous slight Tl hypointensity and T2 hyperintensity with thin mural enhancement following the injection of Gd-DTPA. Ten cases were muhilocular, and 5 showed mild pericystic edema. Conclusion Soft tissue ganglion cyst of the foot and ankle are more common in middle aged women. They are frequently located in the ankle and medial dorsum of foot. On MRI they usually appear as multilocular cysts with homogeneous slightly low signal intensity relative to muscle on T2WI, high signal intensity on T2WI and contrast enhancement of the thin wall.
