BiPAP呼吸机治疗COPD并发型呼吸衰竭28例疗效观察

目的 :研究应用无创性鼻 (面 )罩 Bi PAP通气治疗慢性阻塞性肺病 ( COPD)并发 型呼吸衰竭的临床价值。方法 :对 2 8例 COPD并发 型呼吸衰竭患者常规吸氧、解痉平喘 ,应用激素 ,抗感染等治疗 ,同时给予鼻 (面 )罩 Bi PAP通气治疗 ,观察治疗前后患者临床症状、心率、呼吸频率、血压、动脉血气分析等指标的变化。结果 :治疗前后临床症状明显改善 ,PO2 ,PCO2 ,Sa O2 ,心率及呼吸频率变化比较有显著性差异 ( P<0 .0 5 ) ,血压变化不明显( P>0 .0 5 )。结论 :COPD并发 型呼吸衰竭患者应用无创性 Bi PAP鼻 (面 )罩通气治疗是安全而有效的。

Clinical observation of using BiPAP ventilator in treating 28 cases of COPD patients complicated by respiratory failure

Objective:To study the effect of the mask-to-face ventilation using BiPAP ventilator in COPD patients complicated by respiratory failure .Methods:The 28 cases COPD patients complicated by respiratory failure were treated with traditional methods, plus BiPAP ventilation. The change of the clinical manifestation , heart rate(HR), respiratory rate(RR) ,blood pressure, and blood gas analysis was observed before and after the treatment. Results:The clinical manifestations were notably improved and the values of PO 2,SaO 2,were significantly higher than those before the therapy(P<0.05).The values of PCO 2,HR,RR were lower than those before the therapy(P<0.05),the blood pressure was not changed after the therapy(P>0.05).Conclusion: Using mask- to- face ventilation by BiPAP ventilator is effective and safe to patients with COPD complicated by respiratory failure, and it is worthy of being widely used.
