原发性胆囊癌外科治疗(附34例分析)

目的:探讨胆囊癌的临床特点和提高疗效的途径。方法:对我院8年来收治的34例胆囊癌患者临床资料进行分析。结果:胆囊癌多见于中老年人,平均年龄64.61岁,34例患者中男12例,女22例,男女之比为1:1.83,胆囊癌合并胆囊结石者占65.71%,术前B超诊断符合率为82.14%。手术切除率为73.53%,根治性切除10例,根治率40%。1、3、5年生存率分别为58.82%、8.82%、2.94%。结论:B超、CT是常用的诊断方法,对易患胆囊癌高危人群应进行定期观察。手术方式及治疗效果取决于胆囊癌的分期。

Surgical treatment of gallbladder cancer: clinical analysis of 34 cases

Objective:To evaluate the clinical characteristics of gallbladder cancer and to improve the therapeutic result. Methods: Thirty four cases of gallbladder cancer treated in our department were retrospectively analyed in recent 8 years. Results: The higher incidence was in the elderly. There were 12 males and 22 females,with M:F=1:1.83,mean age 64.61 years, 23 patients(65.71%) were associated with gallstones, 23 (82.14%) caese were diagnosed by preoperative B-ultrasonography. All cases underwent laparotomy with a resection rate of 73.53% and 10 cases(40%)received radical resection. Postoperative 1-,3-,5-year survival rates were 58.82%,8.82% and 2.94%. Conclusion: B-ultrasonography and CT scan are common diagnostic methods. High risk popalation should be observed in time. The type of operation and the result of the treatment are dependent on the Nevins stage and pathology.
