摘    要:目的对比分析DSA与SCTA检查对出血性脑血管病的确诊价值。方法对出血性脑血管病中DSA检查阳性的38例病人与之前所做的SCTA结果进行对比分析,探讨SCTA对出血性脑血管病病因诊断的准确率。结果本组病例中有29个动脉瘤在SCTA影像清晰观察到脑动脉瘤的瘤体大小、瘤颈宽度及与载瘤动脉的关系,与DSA及介入栓塞、动脉瘤夹闭术所见完全吻合,比较两种检查方法对动脉瘤瘤颈宽度、瘤体最长径数值无显著性差异,有2例在DSA检查中未发现SCTA显示部位动脉瘤,检出率为93.55%,其中大脑中动脉M2段1例、大脑前动脉A2段1例;6例动静脉畸形DSA显示清晰,SCTA清晰显示4例脑动静脉畸形血管团大小、形态及供血动脉和引流静脉,检出率为85.71%,1例显示为阴性,此例行DSA检查动静脉畸形位于胼缘动脉末端,属微小的动静脉畸形。结论 SCTA对脑血管病变的诊断既安全、方便、快速且定性、定位明确,可作为脑动脉瘤、脑血管畸形等脑血管病变筛查的首选方法。

Abstract:Objective To compare the value of DSA and SCTA examinations on cerebrovascular disease.Methods38 patients with hemorrhagic cerebrovascular disease were enrolled.The 38 patients were examined by DSA and compare with SCTA examinations.Results 29 cases of aneurysms observed in the SCTA images were completely anastomosis to the DSA images and that seen in the aneurysm surgery or angiographic embolization at cerebral aneurysm size,neck width and the relationship between the parent artery.The aneurysm neck width and the longest aneurysm diameter was significant different.There were 2 cases diagnosed with DSA,but without SCTA.The detection rate was 93.55%.Conclusion SCTA on the diagnosis of cerebral vascular disease is safe,convenient,fast and qualitative,which can be the first choice at the detection of cerebral vascular disease such as cerebral aneurysm,cerebral arteriovenous malformation.
