2型糖尿病合并恶性肿瘤患者心理状况、应对方式与生存质量现况调查及相关性分析

目的 分析2型糖尿病合并恶性肿瘤患者心理状况、应对方式及生存质量的现况，并探讨患者心理健康、应对方式与生存质量三者之间的关系。方法本研究为横断面研究，选取2型糖尿病合并恶性肿瘤患者(恶性肿瘤在糖尿病之后或同时确诊)310例，采用症状自评量表(SCL-90)、医学应对问卷(MCMQ)和世界卫生组织生存质量测定量表简表(WHOQOL-BREF)分别对患者的心理健康状况、应对方式和生存质量状况进行测评。结果 T2DM合并恶性肿瘤患者SCL-90自评量表总分为(179.8 51.2)分，平均分≥2分的前5位的健康因子分别为：躯体化症状、焦虑、抑郁、恐怖、人际关系紧张。患者应对方式的三个因子得分从高到低依次为：面对、回避、和屈服因子。T2DM合并恶性肿瘤患者与常模组相比在生理、心理、独立性、社会关系和总的生存质量与健康状况得分更低（P<0.05）；环境和精神支柱与个人信仰得分则高于常模组（P<0.05）。T2DM合并恶性肿瘤患者SCL-90总分与患者总体生存质量、生理领域、心理领域得分呈正相关；应对方式中，面对因子与总体生存质量和健康状况、生理领域、心理领域得分呈正相关；屈服因子与总体生存质量和健康状况、心理领域得分呈负相关。结论T2DM合并恶性肿瘤患者心理问题主要表现在躯体化症状、焦虑、抑郁、恐怖、人际关系紧张等方面，多数患者采用积极的面对方式，患者生存质量较低。

Correlations among Mental Health,Coping Style and Quality of Life in Type 2 Diabetic Patients with Cancer

Objective:To analyze the characteristics of mental health,coping style and quality of life,and to ex-plore the correlation between mental health,coping style and quality of life in type 2 diabetic patients (T2DM)with canc-er.Methods:This study is a cross-sectional survey.31 0 cases of type 2 diabetic patients with cancer were selected,they were assessed by symptom self-rating scale (SCL-90),The World Health Organization Quality of Life (WHOQOL-BREF) and Medical Coping Modes Questionnaire (MCMQ).Results:The SCL-90 self-rating scale score was (1 79.851 .2)for T2DM patients with malignant tumor.The the top five health factors which average score is higher than or equal to 2 points are body symptoms,anxiety,depression,phobia,and interpersonal relationship tension.The three factors of the coping style of the patients were from high to low:face it,evade,and surrender.T2DM patients with malignant tumor patients were lower in physiology,psychology,independence,social relations and overall quality of life and health status scored lower (P<0. 05 )than normal control group;environment, support,and the personal beliefs score is higher than that of the normal control group (P<0. 05).In T2DMpatients with malignant tumor SCL-90 scores was positively related to the o-verall life quality, physiologicalscore, and psychological scores;coping style,face factor was positively related to the overall quality of life and health,physical score,psychologi-cal scores;surrender factor was negatively correlated to gen-eral quality of life and health status,psychological score.Conclusion:The psychological problems of patients with T2DM complicated with malignant tumors were mainly manifested in the aspects of somatic symptoms,anxiety,depression,terror, interpersonal tension,and so on.
