去除人类冷冻胚胎中坏死细胞对其移植结局的影响

摘    要:目的:评价人类冷冻胚胎解冻后,在胚胎移植前,采用激光辅助孵化及吸出胚胎的坏死细胞碎片是否会提高胚胎的种植率。方法:回顾性的分析了91个冷冻胚胎移植周期的临床妊娠率和胚胎植入率,其中4 1个周期(A组)在胚胎移植前,采用激光辅助孵化结合显微操作去除冷冻胚胎中的坏死细胞碎片,5 0个周期(B组) ,采用激光辅助孵化,但没有去除坏死细胞碎片。采用慢速冷冻 快速解冻的方法。结果:在冷冻胚胎移植前去除胚胎的坏死细胞碎片,能显著性地提高移植周期的临床妊娠率(43.91%vs.2 4 .0 0 % ,P <0 .0 5 )和胚胎植入率(19.4 5 %vs.10 30 % ,P <0 .0 5 )。结论:在人类冷冻胚胎移植前,采用激光及显微操作辅助去除冷冻胚胎的坏死细胞碎片,能有效提高胚胎的种植率。

Abstract:Objective:To ev aluat e the effects of laser-assisted drilling an d micromanipulation removal of necrot ic blastomeres from partially damaged cryo p reserved embryos on embryo implantation ra te. Methods:We retrospectively an alyzed the pregnan cy rate and implantation rate of 91 frozen e mbryo transfer cycles,including 41 cy cles of which necrotic blastomeres were re moved from partially damaged human frozen- thaw ed embryos with technique of laser-assiste d drilling and micromanipulation extra cting and 50 cycles,from which necrotic bla stomeres were not removed,but still had l aser-assisted drilling.On day 2 or 3 aft er oocytes pick-up.Embryos wer e frozen by using a slow cooling procedure,and then the fast thawi ng protocol was carried out. Results:Removal of necrotic b lastomeres from par tially damaged frozen-thawed human embryos before transfer significantly increas ed the clinical pregnancy rate (43.91% vers us 24.00%) and the implantation rate (19.45% versus 10.30%) compared with the co ntrol group. Concluslons:The implantation potential of pa rtially damaged human frozen embryos can be improved by laser-ass isted drilling and micromanipulation remov al of necrotic blastomeres.
