内镜治疗重度食管静脉曲张破裂出血的临床研究

目的:比较内镜下硬化剂、套扎、套扎 硬化剂序贯法治疗重度食管静脉曲张破裂出血的近期疗效和安全性.方法: 对 59例肝硬化并重度食管静脉曲张破裂出血的患者进行随机分成三组,分别接受硬化剂、套扎、套扎及硬化剂的序 贯疗法.结果:近期止血成功率、平均治疗次数、再出血率及病死率三组均接近.但在严重并发症上,硬化剂治疗组 明显增多,套扎及硬化剂组次之;半年后曲张静脉的复发率以套扎治疗组明显增高,余下两组相近.结论:硬化剂、套扎、套扎及硬化剂的序贯疗法是目前治疗重度食管静脉曲张破裂出血的有效且经济的方法.

To investigate the clinical result of sclerotherapy,banding ligationand sclerotherapy plus banding ligation sequential treatment in severe as ophagealvaricealbleeding

Objective To investigate safety and efficacy of the treatment in severe esophageal variceal bleeding with sclerotherapy,banding ligation and ligation+ sclerotherapy sequential treatment. Methods 59 cirrhosis patients with severe esophageal variceal bleeding were randomly divided into three groups, all patients received sclerotherapy,banding ligation and sclerotherapy + banding ligation sequential treatment respectively. Results Recent hemostasis rates, average times of treatment, rebleeding and death rates are all closely eaqual,as to the rate of severe complications, the group of sclerotherapy is higher than the group of banding ligation and banding ligation + sclerotherapy. The recurrence rate of esophageal varice in half- year follow- up, the group of simple ligation is higher than the other two groups. Conclusion Sclerotherapy, banding ligation and sclerotherapy + banding ligation sequential treatment are all effective and economical to severe esophageal variceal bleeding at present.
