自身抗体联合检测诊断系统性红斑狼疮的价值

目的通过检测系统性红斑狼疮（SLE）患者的抗核抗体（ANA）及抗核抗体谱（ANAs）,评价自身抗体对SLE的诊断价值及临床意义。方法 SLE组、疾病对照组的所有患者血清标本均采用间接免疫荧光法检测ANA和免疫印迹法检测ANAs。结果 SLE组ANA、抗nRNP/Sm、Sm、SSA、Ro-52、CENP B、dsDNA、AnuA、Histone、rRNP检测阳性率均高于疾病对照组（P〈0.05）,抗CENP B检测阳性率低于疾病对照组（P〈0.05）。9种自身抗体的相关参数,ANA的敏感性最高,但特异性低,阳性似然比低。抗Sm、dsDNA、AnuA、rRNP的敏感性较低,但特异性高,阳性似然比大于8。阳性似然比大于8的4种自身抗体的互补关系,抗Sm阴性时抗dsDNA、AnuA、rRNP的阳性率为39.38%、33.75%、13.13%。但2项联合（AnuA＋dsDNA）、3项联合（AnuA＋dsDNA＋Sm）、4项联合（AnuA＋dsDNA＋Sm＋rRNP）检测时敏感性、阳性似然比和诊断准确性均高于单项ds2DNA（P〈0.05）,阴性似然比均低于单项dsDNA（P〈0.05）,而特异性无明显变化（P〉0.05）。结论抗dsD2NA、AnuA、抗Sm、抗rRNP是SLE诊断的标记性抗体,特异性高,但敏感性低,若联合检测可以避免因单项检测出现的漏诊,提高对SLE诊断的敏感性。

Significance of Combined Detection of Autoantibodies in the Diagnosis of SystemicLupus Erythematosus
