甲硝唑、阿莫西林、奥美拉唑联合治疗幽门螺杆菌阳性的消化性溃疡

目的　评价以奥美拉唑、甲硝唑、阿莫西林为核心的三联疗法治疗幽门螺杆菌 (HP)阳性的消化性溃疡的疗效。方法　对内镜诊断的 85例HP阳性的消化性溃疡患者进行随机分组对照观察。奥美拉唑组 43例 ,2 0mg 1次 /d :联合用药组 42例 ,奥美拉唑 2 0mg 1次 /d ,甲硝唑 2 0 0mg 4次 /d ,阿莫西林 5 0 0mg 4次 /d ,疗程均为 2周。观察指标包括溃疡愈合情况 ,疼痛缓解情况 ,HP根除率及年累积复发率等。结果　服用奥美拉唑组和联合用药组溃疡愈合率分别为 93 0 %和 95 2 % (P >0 .0 5 ) ,疼痛消失平均时间分别为 4.0d和 3 .5d(P >0 .0 5 ) ,HP根除率分别为 48 8%和 95 2 % (P <0 .0 1) ,年累积复发率分别为 48%和 18 8% (P <0 .0 5 )。结论　奥美拉唑、甲硝唑、阿莫西林联合用药治疗HP阳性的消化性溃疡 ,溃疡愈合率高 ,疼痛缓解决 ,且HP根除率和年累积复发率均显著地低于单用奥美拉唑。

Clinical Study on MOA Triple Cure in the Treatment of the Peptic Ulcer of Pylorus Helicobacter Positive

Objective To evaluate the therapeutic effect of omeprazole, metronidazole and amoxicillin triple cure on the peptic ulcer of pylorus helicobacter positive. Method there were 85 patients of the peptic ulcer of pylori helicobacter positive by Endoscopic diagnosis. The patients were divided into two groups at random: Omeprazole group: 43 patients, Omeprazole 20 mg qd; Triple cure group 42 patients, Omeprazole 20 mg qd, Metronidazole 200 mg qid. Amocillin 500 mg qid. After the patients were treated for two weeks. To observe peptic ulcer heal, bellyache palliative, Hp eradication rate and recurrence rate. Result In omeprazole group and triple cure group: The peptic ulcer heal rate was 93.0% and 95.2% respectively ( P >0.05). bellyache palliative mean time was 4 days and 3.5 days respectively ( P >0.05). Hp eradication rate was 48.8% and 95.2% respectively ( P <0.01). recurrence rate was 48% and 8.8% respectively ( P <0.05). Conclusion By omeprazole. metronidazole and amoxicillin triple cure in the treat of the peptic ulcer of pylorus Helicobacter positive, Ulcer heal rate is high, bellyache palliative quick, moreover Hp eradication rate and recurrence rate has significant decrease than omeprazole group. It is necessary to increase the MOA triple cure for treatment of the peptic ulcer in clinic.
