阴茎根部压迫试验在诊治血管性勃起功能障碍中的作用

目的：对比彩色多普勒超声（CDU）评价阴茎根部压迫试验（CTPB）在诊治血管性勃起功能障碍（ED）中的作用。方法：对69例拟血管性ED患者施行CTPB和CDU，统计CTPB诊断正确度；总结18例阴茎背深静脉包埋术患者术后1年的满意率，比较术前有无CTPB诊断对患者术后满意率的影响。结果：69例ED患者CTPB中阴茎动脉正常35例，不正常34例，CDU中阴茎动脉正常37例，不正常32例，统计提示CTPB诊断血管性ED正确度较高（Youden指数=0．731）。CTPB和CDU共同诊断为静脉性ED的10例，术后1年疗效满意9例（90％），单纯CDU诊断为静脉性ED8例，术后1年满意3例（37．5％），两组疗效满意率有统计学差异（P〈0.05）。结论：CTPB诊断血管性ED有较高正确性。CTPB和CDU的互补可提高血管性ED诊断正确性，从而提高手术疗效。

The Value of Compression Test of Penile Base in Diagnosing and Treating Erectile Dysfunction

Objective To compare the value of compression test of penile base(CTPB) with color Doppler ultrasonography( CDU ) in diagnosis and treatment of vascular ED. Methods 69 cases of vascular ED received CTPB and CDU; 18 cases of venous ED underwent the operation of embedding deep dorsal vein of penis and the quality of sex life in postoperative follow-up was 1 year. Results Of the 69 patients of vascular ED, 35 cases attained better erection in sexual stimulation and 34 cases were unable to achieve erection with CTPB, 37 cases had normal PSV values of the cavernous artery and 32 cases showed low PSV values with CDU. We studied the coincidence of CTPB and CDU in diagnosing vascular ED (Youden index = 0.731). Among 18 cases of venous ED, 10 cases diagnosed by both CTPB and CDU had higher postoperative satisfaction rate(90%) than 8 cases that were diagnosed by CDU alone(37.5%), the difference was significant in satisfactory rate(P <0.05). Conclusions CTPB has a high coincidence with CDU in diagnosing vascular ED. Combining CTPB and CDU will elevate the high selection for operation and improve the operation result.
