左半侧结肠癌并完全性肠梗阻的治疗

目的　探讨左半侧结肠癌并完全性肠梗阻的治疗方法。方法　分析近四年来手术治疗 14例左半侧结肠癌并完全性肠梗阻的临床资料。结果　 14例左半侧结肠癌并完全性肠梗阻患者中 9例在术中作了满意的结肠减压灌洗后行一期切除吻合术 ,术后 1例发生切口感染 ,无吻合口瘘发生。 5例因全身情况差作肿瘤肠段切除、近端结肠造瘘 ,有 4例发生切口感染。均痊愈出院 ,无围手术期死亡。结论　严格挑选病例 ,通过术中满意的肠道减压灌洗 ,左半侧结肠癌并完全性肠梗阻一期根治切除吻合是安全可行的

The treatment experience of left hemicolon cancer with complete obstruction

Objective To discuss the therapeutic method for left hemicolon cancer with complete obstruction.Methods 14 cases of left hemicolon cancer with complete obstruction who had been operated during past 4 years were analyzed.Results 9 cases out of the 14 were operated with one stage resection and end to end anastomosis after satisfactory colon depressuring and lavage,only one patient suffered from wound infecting without anastomosis fistula.5 patients were operated with neoplasm segment resecting and proximal colon fistulization,4 of whom were striken by wound infection.But all the patients recovered favorably and the perioperation mortality was zero.Conclusion One stage resection and end to end anastomosis can be considered as safe and feasible,if we strictly select the patients and satisfactorily colon depressuring and lavage beforehand.
